In a state of health, erythrocytes float through the bloodstream as single entities bouncing off each other when contact is made. PEMF: Bone Formation and Fracture Healing
PEMF Therapy has been used for decades to expedite the healing of bone fractures. In 1979, PEMF therapy was approved by the FDA for application in the healing of fractures that have proven unresponsive to traditional therapies. These are commonly referred to as nonunion fractures.
Again, the role of PEMF Therapy in accelerating the process of osteogenesis can be extrapolated down to maximizing cellular function. Putting aside essential factors such as stabilization, alignment, infection control, etc. let’s focus on what happens at the cellular level during the healing of a bone fracture.
Healing Process of Bone
The healing process of a bone fracture is a highly complex physiological process which involves the coordinated functions of multiple specialized types of cells.
- Immediately following the occurrence of a bone fracture, the body begins the healing process. Initially, a blood clot is formed around the fractured area. The matrix of this clot is filled with cells from the immune system called phagocytes. These cells absorb traumatic debris and bone fragments from the injury site; they also work to eliminate any germs or bacteria that may have entered the site of the fracture.
- After the clot formation is complete, another set of specialized cells begin their work. Chondroblasts commence forming a soft collar at each broken end of the bone. This soft collar, commonly referred to as a callus, is mostly comprised of collagen fibers. Chondroblasts produce these collagenous fibers and they grow until each end meets, forming a complete unit that binds the fracture together.
- As the healing process continues, this collagen-based callus will be replaced by a porous substance called trabecular bone. Trabecular bone is highly vascularized and has a concentrated capacity to deliver oxygen and nutrients to the healing zone of the fracture.
- This process of laying down the early phases of new bone growth provides stabilization of the fracture and the initial structural form of what will eventually be fully calcified bone.
- In the final stage of fracture healing, the bone is remodeled. Osteoblasts are comparatively large specialized cells fundamentally responsible for this final stage of remodeling. Although they are present during other phases of healing, their role is most significant in synthesis and mineralization of bone during this final stage of reconstruction.
- Osteoblasts form a tightly compressed sheet on the surface of the bone.
This layer of cells works to extend cellular processes through the entire area of newly developing bone.
- Another group of cells called osteoclasts work during this phase to break down extra bony tissue left behind by the callus until the healing process is complete and the bone is returned to its original shape.
- Bone remodeling is a gradual process and can take up to 10 years to reach full completion. Factors such as age, general physical health, and relative to our topic – cellular health and level of cellular function.
